  1. Return the blue St. Anthony Village High School eligibility form (must have signature of parent / guardian and athlete).
  2. Sign and return the Minnesota State High School League Athletic Eligibility Statement (turquoise form).  Parent / guardian and athlete must sign.  The Parent’s Permit and Health Questionnaire on the backside of the turquoise sheet must also be filled out and signed by a parent or legal guardian.
  3. A physical exam is needed every three years.  If your son / daughter have not had a physical in the past three years, they must have a physical on record at SAVHS before they may begin participation.  For questions regarding records of physical examination please contact the SAVHS nurse’s office at 612 706-1107.
  4. The participation fee for your sport must be paid before athletes are allowed to start practice.  Please see the correct fee for your sport in the table on page 2.  Checks are to be made payable to St. Anthony Village High School.  If you qualify for the lunch program, the athletic fee is waived.  You need to sign the attached tan sheet in order for the school district to release the information to the athletic office that you qualify for the lunch program.
  5. The St. Anthony – New Brighton School District does not provide athletic insurance nor does it recommend any specific insurance carriers who might cover athletes.  The responsibility for insurance lies with individual families.
  6. Each coach carries an Emergency Card for your child in case of an emergency.  Please fill it out and return.

***There is a maximum total fee of $530.00 per family per school year.  Fees may be waived for students who qualify for free or reduced lunching.  Download the free and reduced application here .

Refund of Athletic Fees - Athletic fees will be refunded in full to students who drop the activity before the first official contest in that sport.  One-half of the fee will be refunded if the student drops before the 3rd official contest.  No refund will be given following the 3rd official contest.

Students who drop the activity for medical reasons will be handled on an individual basis.

Refunds will not be given until an equipment clearance is issued by the coach in charge of the sport.  Refund checks will always be issued to the parent of the athlete.
